Community Geriatrician Service

The Community Geriatrician Service aims to provide access to senior citizens that are homebound due to a physical health problem, obtain a comprehensive Geriatric Assessment. Accordingly, the community geriatrician will carry out a domiciliary medical visit together with professionals from the CommCare Assessment Unit.


Following referral, the case will be discussed between the community geriatrician; the CommCare team and the patient’s general practitioner. After such a consultation, a home visit would normally follow, particularly when dealing with complex medical issues requiring specialist geriatric management. Other professsionals from the CommCare multidisciplinary team may also be involved, with follow-up visits according to the patient’s needs.


General eligibility criteria:

  • A senior citizen over 60 years of age
  • Living in the community
  • In need of a non-acute intervention
  • Unable to go out of home and attend a geriatric outpatient clinic


When a nurse or a professional member of the Allied Health Professions from CommCare identify the need for the Community Geriatrician Service, the patient’s general practitioner will be advised to complete a Ticket of Referral. This referral will be received by a designated key person who will discuss the case with the Community Geriatrician.
